Clinic

WEAR PROFESSIONAL CLOTHES.
  • Don’t have to prep clinic in advance.
  • Patients complete surveys in advance.
  • Categorize patients’ symptoms — urinary, bowel, sexual.
  • Use templates (dot phrases in orientation doc).
  • Dot phrases — can take from Alex Sundermann or Amie Kawasaki. Kisby, Kawasaki.
  • Problem-based charting → steal dot phrase .DAN from Kawasaki for A/P.
  • POP-Q exam resource: https://pop-q.netlify.app/ — enter exam results to get stage of prolapse.
  • You will get to do this exam in clinic and are expected to do it.

ORs

Spend most time there.

  • Meet patient and med student in advance.
  • Prep patients in OR with position / prep.

Cases:

  • If vaginal surgery, don’t do as much.
  • If TVH, expected to do most of it. Kisby does special entry → Mayo Clinic style with anterior entry transvaginally (must prep and know steps prior to surgery). See email Kisby sent out with resources on Mayo TVH.
  • Overall, KNOW STEPS FOR TVH Day 1 (non-Mayo method if not working with Kisby).
  • If robotic, do entry and bedside assist. If modules and bedside completed, can sit at console.
  • Fellows write all the op notes, unless you do the entire case (i.e., Sling, etc.) — but uncommon.

PGY-2 Job:

  • Post-op orders.
  • Patient discharge instructions.
  • Admission orders (admit to obs).
  • Send “email” in Epic including patient void trial results. Communicates patient disposition based on void trial (foley vs self cath). Fellows look for it all night until result sent.
  • Lots of anterior cystocele and posterior rectocele repairs — helpful to review anatomy.
If you operate and finish a case before 2 PM, the expectation is that you go to PM clinic — professionally dressed.

Rounding / Consults

Wait to see if something comes through.

  • If patients stay, call night team for sign out after last case or ~7:30 PM (text night team for good sign out time).
  • For night team want everything done for them — so stay and do all the orders.
  • If people don’t stay, no rounding.
