- •72-hour consent (medical and surgical) — can be signed by RN, MD, PA, NP; every page needs a sticker; write “Duke University Hospital.”
- •Hematocrit (>30).
- •Talk about future contraception.
- •Schedule surgical (usually nurses have done this already but check).
Pre-procedure rules:
- •NPO 8h, clear liquids OK up to 2h pre-op.
- •For moderate sedation visits: .fpmodsed in the Wrap Up tab.
- •Make a copy of consent (original + patient copy).
- •Medical vs surgical consent differ only in the risks table (infection/bleeding higher with meds) — frame as “in case you change your mind or need the other.”
Instructions / forms:
- •Green forms — for med Ab.
- •White forms — for mifepristone consent and then outside-of-state form.
- •Purple forms — for surgical Ab.
- •Attending signature form for both medical and surgical abortions.
